    Job Summary:

     

    The Discharge Planner / Case manager counsels and aids individuals in the program in dealing with substance and alcohol abuse and recovery.

     

    Reports To: Chief Clinical Officer / Clinical Director

    Responsibilities

    Job Duties

    • Discharge Planner/Case Manager will meet with patient within 24 hrs and obtain Release of Information (ROI)/HIPPA and document meeting with patient chart.
    • Discharge Planner/Case Manager will schedule a 1:1 with patient after treatment planning meeting and begin a recovery plan. The plan is to coordinate continued care treatment upon discharge and should be documented in patient’s chart.
    • Discharge Planner/Case Manager will continue meeting with patient in a as needed basis to ensure continued care is arranged ahead of time.
    • Discharge Planner/Case Manager will ensure arrangements for continued care is set up as close as possible to patient discharge. This will include, but not limited to intensive outpatient appointments, halfway house interviews, transfers to other facilities, and any other recommendations.
    • Discharge Planner/Case Manager will arrange transportation for patients, as needed.
    • Discharge Planner/Case Manager will attend morning rounds and all scheduled treatment planning meetings and actively participate as part of the treatment team.
    • Discharge Planner/Case Manager is expected to communicate with the treatment team on update information regarding placement.
    • Discharge Planner/Case Manager will be required to complete the discharge forms as required by Sunrise House/American Addiction Centers to present to the primary counselor for their discharge session with patient.
    •  Discharge Planner/Case Manager is expected to complete all questionnaires and surveys that are required by Sunrise House/American Addiction Centers.
    • Discharge Planner/Case Manager will contact referrals, EAP’s, business development representatives for recommendations for continuum of care, on a as needed basis.
    • Attend all required in-services trainings and clinical department meetings.
    • Complete all required trainings through Health Stream Series.
    • Maintain compliance with all Sunrise House/AAC policy and procedures.
    • Maintain compliance with all Division of Mental Health and Addiction services (DMHAS) regulations and Commission on Accreditation of Rehabilitation Facilities (CARF) standards.
    • Other duties assigned in the scope of the job title.

     

     

    Qualifications

    Job Requirements:

    • High School Diploma required ; Bachelor’s Degree preferred.
    • Two years’ experience in case management in a Hospital or Clinical environment.
    • Dual Diagnosis background preferred.
    • Drug & Alcohol counseling certification from an accredited institution.
    • Two years’ experience working in a substance abuse environment.
    • Ability to work effectively with people and aid them in adjusting to their specific problems.
    • Ability to establish and maintain effective working relationships with clients, family members, and other members of staff.
    • Knowledge and experiences in running groups, meetings and/or activities.
    • Ability to read and interpret written information; write clearly and informatively; edits work for spelling and grammar. Ability to speak clearly and persuasively in positive or negative situations; listens and gets clarification; responds well to questions; demonstrates group presentation skills; and participates in meetings.
    • Working knowledge of Microsoft Word, Excel, and Outlook.
    • If in personal recovery, a minimum of 2 years clean and sober.
