Job Summary
Responsible for providing individual, group and family counseling. Conduct clinical assessments for new patients and on-going required assessments, conduct Phase 2 assessments, develops treatment plans, sets up aftercare appointments, assess for outcome measures, and develops safety plans, completed discharge paperwork for patients in rehab level of care, while providing daily individual support and group-counseling services. Counselor II will demonstrate expertise in therapeutic interventions, treatment modalities and utilize evidence based practices. Counselor II will participate in Treatment team and provide service coordination as needed for patients. Counselor II will advocate and communicate with the Utilization Review department on all aspects of patient care. AAC/AdCare strives to ensure that all of our patients receive the most comprehensive care to address the issues that may impede their recovery. Counselor II ensures that their support is consistently delivered in accordance with AdCare’s clinical design specific to the program and is delivered in a manner that can demonstrate sound outcomes reducing relapse from alcohol and other drugs. This position fosters an atmosphere of customer service and excellence in service delivery. Provides personalized and tailored support to address the individual needs of our patients by consistently adhering to the mission and values of AdCare and AAC. Has a deep knowledge for ASAM criteria to build treatment plans and justify length of stay. Able to communicate clearly and effectively when participating in daily treatment team meetings with the multidisciplinary team. Understands levels of care, aspects of case management, utilization review, and placement programing. Skilled in de-escalation and crisis management. Conducts psychosocial assessments, develop treatment plans and provide both individual and group, and family counseling services for every patient on the caseload.
