Job Summary


This position is the first point of contact for the Medical Records department. This position receives and facilitates requests for release of patient information Multiple function are performed by the medical records coordinator and allows for medical records and information to flow throughout the AdCare and AAC system of Care.


Duties and Responsibilities

  • Answers department phone calls and routes to the appropriate destination.
  • Performs various general clerical duties; including faxing, filing, scanning and emailing.
  • Assists walk-in patients with requests.
  • Manages correspondences for patient records.
  • Reviews paperwork received from inpatient and outpatient departments after patient is discharged to determine the need for inclusion in the record. If necessary, information is scanned into Electronic Medical Records (EMR).
  • After confirming record contains appropriate release information, prints reports from the CPSI EMR system and sends to primary care physician listed on patient record.
  • Identifies, investigates and resolves issues related to completeness and errors in medical records.
  • Screens chart locations to verify that relocating rules are followed.
  • Files all patient records accurately and timely.
  • Purges records from the main file area according to the timetable established.
  • Catalogs and sends the purged records to contracted document storage facility.
  • Runs reports weekly and monthly on administrative discharges and discharges against medical advice.
  • Follows all requirements of HIPAA,42 CFR Part 2, and state laws regarding protected health information.
  • Provides coverage when other department members are out.


  • Minimum High School Diploma required.
  • Minimum 1year required experience working in a medical office or medical records department.
