Responsibilities

Job Summary

 

Assistance with clinical tasks throughout the outpatient system and care management for clients.

 

Duties and Responsibilities: .

 

  • Meet with individual patients weekly or more throughout course of treatment to discuss any current needs, provider referrals to community collaterals
  • Assist patients in identifying and linking to appropriate resources for immediate needs and for the next levels of continuing care, safe and sober housing, and other recovery supports; assist patients with interviews and visits to prospective programs and providers
  • Attend staff meetings, case conferences and required training to coordinate with the program team and ensure quality of care and continuity in accordance with clinical policy
  • An additional person to assess client satisfaction and address concerns such as schedule changes, making appointments, answer questions regarding their course of treatment and level of care.
  • Assistance with conversion and follow up from intake to IOP, and transitions of level of care as well as completion of TEA, Patient Satisfaction Surveys
  • Compilation of collateral contact information, establishing systems to ensure regular, consistent communication to outside collaterals. Facilitate this process for clinicians (i.e. recovery court, DCF, probation).
  • This includes answering phone calls to clients requesting treatment updates, etc.
  • Assistance for clinicians in management of treatment team review.
  • Filing/billing assistance
  • Treats patients, and colleagues with dignity and respect; honors each person’s right to privacy/maintains very strict patient and colleague confidentiality; respects all individual differences; supports cultural diversity.

Qualifications

Education/Experience:

  • Bachelor’s Degree in Behavioral Sciences such as Human Services, Social Work, Psychology or other related field required. 
  • Experience in a behavioral healthcare field and/or substance abuse recovery field.
  • Experience in behavior health treatment including;as well as case management for individuals diagnosed with a substance use and/or mental health disorder.
  • Knowledge of ASAM criteria.
  • Ability to read, write and interpret written information, type documentation, present information to groups and participate in meetings.
  • Working knowledge of Microsoft Word, Excel, and Outlook.
