Job Summary

Support the chart management activity and medical records requests within the Health Information Management Department. This position is also responsible for working with patients to complete filing for disability and FMLA benefits.


Duties and Responsibilities:

  • Maintenance of the master patient index.
  • Digitize paper documentation of active and discharged patient charts.
  • Reviews and completes requests for medical records to include presence in treatment letters, completion of treatment letters as appropriate
  • Ensures all requests have proper releases of information forms signed and completed prior to any communication or correspondence. Ensure all external releases meet criteria for transmission of SUD records.
  • Completes any post-discharge requests of patients, external agencies, HCP, legal organizations, record retrieval companies.
  • Document communications as required and scan any corresponding paperwork.
  • Reviews chart documentation for completion and interpretation of services rendered.
  • Performs chart reviews and pulls for billing requests.
  • Actively works with the business office on invoicing of outgoing requests.
  • Performs quantitative and qualitative audits of records as needed.
  • Actively communicates with staff on any missing or incomplete documentation of recently discharged patients, or records requiring transmission during patient stay.
  • Maintains master index of all records held by adjoining records management company.
  • Develops annual list of record purge for facility, records management company and updates master patient index.
  • Mails, Emails, or faxes medical records in accordance with privacy and HIPAA laws.
  • Attend department meetings and trainings as required.
  • Completes state mandatory reporting of patients admissions, clinical data, and discharge per NJAC1-:161A/B-3.4.
  • Assist Treatment team in the timely completion of appropriate paperwork with regards to Short-Term Disability Claims, Family Medical Leave Act, and Return to Work documentation for active and discharged patients.
  • Supports and promotes an environment of Customer Service.
  • Other duties assigned in the scope of the job title.
  • Assist Quality Assurance Director with projects to promote the continued development and training of documentation programs.


Education/ Experience:

  • Bachelor’s Degree required.
  • RHIA/RHIT or applicable HIMS-related certification preferred.
  • 2 years of experience working with medical records
  • Excellent organizational skills.
  • Good verbal and written communication skills.
  • Ability to maintain patient confidentiality.
  • Ability to maintain the security of all patient records.
  • Working knowledge of Microsoft Word, Excel, and Outlook.
